A 65-year-old woman presented with complaints of a mass descending per vagina, associated with discomfort and difficulty in daily activities. She had a prior history of hysterectomy, following which she developed vault prolapse.
On clinical examination, there was significant post-hysterectomy vaginal vault descent. After evaluation and counseling, the patient was planned for abdominal sacrocolpopexy, which remains the gold standard for apical prolapse repair.

Intraoperatively, the vaginal vault was identified and dissected. A synthetic mesh was placed and anchored to the anterior longitudinal ligament over the sacrum, providing strong apical support. The mesh was then secured to the vaginal vault, restoring normal anatomy.
The procedure was completed successfully with good anatomical correction. Postoperative recovery was uneventful, and the patient experienced significant symptomatic relief.

Abdominal sacrocolpopexy offers durable and effective repair for vault prolapse, with excellent long-term outcomes and restoration of pelvic anatomy.
